Double feed that wouldn't clear

I tried fixing it five different ways but I ended up having to take the whole gun apart. What makes a gun double feed so badly that it won't clear the normal way?

Usually a combo of bad mags, weak springs, or extractor issues. Once tolerances stack wrong, it turns into a real mess fast
